Primary school health questionnaire form

Primary school health questionnaire form - received it in the post this week from the school, is it given to everyone and at this time of the year? I thought it was before starting Reception, my child will be finishing Reception soon.

Well there will be a lot of children who have different conditions, and / or different medications, and/or different needs by the time they leave school, compared with when they were 4 and hadn't started.

It would be a bit odd if any organisation that were caring for your child (or any people) didn't update their records on a fairly regular basis.

Not sure why they've posted them rather than giving them to parents though.

@UsingChangeofName thanks for your reply, teachers are anyways aware of any medications/ conditions if the child has gone to Reception in the same school.? My child had joined 1 month late in Reception so I am not sure if I ever filled this specific form before but definitely filled in some generic form where it asks questions about health.
Do they give this every year? They are aware we are moving places after Reception so that's why thought what was the necessity.
