De-registering letter

I'm de-registering my DD, I just wondered how to word the letter if anyone can help?

Do I have to give the reason?

You need to say if you are home schooling or moving to another school.
Make sure your home address and telephone number are clearly stated as you will be contacted if home schooling.
If moving to another school that school will be contacted.

Some local authorities have an in-year transfer form, which needs to be signed by your current headteacher. If you are withdrawing from school altogether, you need to make this clear to the school as they remain responsible for your child until they arrive in their new school, or are officially recognised as being home educated.

No you don't have to give a reason. If you're deregistering from a mainstream primary school in England, it's an instruction not a request and they must remove from the roll.

Thanks, it's a primary in Wales but I've been told it's the same as England
