Fathers contact details

Hi my DD is starting reception in September and I'm filling out the contact information. It is asking for the fathers details but I don't know wether I should put them on. He has only been in her life consistently for the last year and lives 80 miles away so only sees her once a month for the weekend. He is on the birth certificate.

Should I add him or leave him off?

If he is currently in her life then I’d add him, if only as a back up contact for emergencies

When DD started primary school we had a "Anyone who holds PR" form and then a list of people to contact in an emergency, my Ex lives within a mile of us but sees DD EOW so was advised to only put him on the PR form.

It's caused no issues, she's in year 1 now and the few times someones been contacted they've always got hold of me.
