I should have applied a few years ago but keep putting it off. What exactly do i need to do or collect as evidence? Is there a set format/layout for a file? Where should i start? All im being told is that it goes on my last 2 yrs proffessional development reviews.
Can anyone help?

I'm not sure, but I think you might have missed the boat as new appraisal procedures come in this September, and I think performance increases are going to be based on school/local criteria.
