Preschool assistant application form - where can I find one?

My preschool needs to find a new assistant but need to produce an application form. Any ideas?

I am staffing officer at the local playgroup. Are you members of the PLA? They have a manual which has standard forms in the back - we just photocopied it. If you don't have the manual their website may be able to help.

We also have a pack from the local authority which has a section on recruitment and retention and there is a model in there. Maybe approach your early years advisor and see if they have something you could use.

Thanks. I think they have a pack from the PLA but it isnt accessible to me at the moment as the committee is changing hands. I also wanted something soft copy so I could put the preschool logo on it if possible.

I'm not sure tbh. I've just been asked to write an application form asap and I havent had chance to see what resource we have.

I could pick up a hard copy of one tomorrow possibly, would that help?
