Am I experiencing an ectopic pregnancy?

Hi, so , this will be slightly long, but any advice or words of wisdom would be hugely appreciated. 2 years ago I got the Mirena coil fitted. Without going to off topic, this worked great for me for 18 months but after 6 months my periods returned and something just didn’t feel right.

I missed my last period, took a test, and to my surprise was positive. I’m only young and in no position to have a child, so made the hard decision to look towards an abortion. I attended an appointment where they raised concern as they were unable to see the pregnancy.

I was referred to hospital and had both an external and internal ultrasound. They could see a patch of liquid in both my uterus and my right side ovary. They did not mention the liquid in my ovary however. They asked to do an examination which they did, and removed my coil (I’d just like to add here that my coil strings were cut way too short and they really struggled to remove it). When she did something, I believe she said she was moving my cervix slightly, I had agonising pain in my lower right side. So bad that I shouted out and cried immediately. Agony.

They again raised concern that this was a warning sign of an ectopic pregnancy and requested a round of bloods. I had a second round and they compared the two and my HCG levels had increased, not doubled, but gone up by around 70%.

I was then asked to come back again, which I did today, and had another scan done, which showed the liquid in my uterus (blood) which the Dr said he was not concerned about. He noted at this point about the liquid in my right ovary which had been there, I just wasn’t aware of it. It was identical to how it had looked in the previous scan, however there was only a matter of days between these, so even if it was a pregnancy, there likely wouldn’t be much if any growth at all.

Bouncing back to yesterday, I had some bleeding but just as I wiped and not proper red. Quite watery. This continued slightly today. Until now, where I have had a relatively large bleed, nothing CRAZY but a fair few wipes of proper blood. I did take a picture but didn’t want to gross anybody out! I called the hospital and explained the Dr said today he would expect to see bleeding as he can see it in my uterus, however I was told to call if I’m bleeding.

They had my blood results back and my HCG is still increasing, so they don’t believe I’m miscarrying, but won’t give me any further information.

I’m really at wits end now and just want to know if I am experiencing the start of an ectopic pregnancy.

Does anyone have literally ANY idea or a similar experience they could share? I’m obviously quite concerned and not getting much from the hospital other than ‘we’re just not quite sure yet’. Any help would be HUGELY appreciated.

Thanks in advance!

TLDR: Got pregnant on IUD, can’t see pregnancy in uterus, patch of blood in uterus, unidentified liquid in right ovary, HCG levels still increasing.

It does sound like it could be ectopic.
Ur cool would have put you at increased risk of this.
At the moment it sounds like they are following "pregnancy of unknown location" protocols.
Once ur HCG gets above 1500 they would expect to see something/sac on a scan.
I had a ruptured ectopic in February and it took 6days between presenting with an excruciating pain that lasted only minutes followed by a small amount of watery brown blood.
I did have some discomfort when trying to go to toilet but that passed too.
1st scan showed free fluid in abdo, HCG climbed over 66%. 2nd scan/bloods 48hrs later showed same, HCG continued to climb but still couldn't see anything. 3rd bloods only climbed 60% so they scheduled exploratory laparoscopic surgery where they found the rupture and internal bleeding.
Pregnancy and tube removed.
Hope it's a different story for you but it does sound familiar.

Well, if anyone ever stumbles across this, and related to me, it was an ectopic pregnancy unfortunately. I just had methotrexate, so fingers cross it’s a success! Thank you for replying to me!

@DeftBear i hope things are ok, and I wanted to ask how your experience ended up with methotrexate? I am 2 weeks on from my injection and just in so much pain still!

Strawberryshortcake14 · 13/08/2024 08:18

I'm sorry it turned out to be an ectopic pregnancy, I have been through one in 2011. I just wanted to share this link ectopic.org.uk/ they've got great support and information should you need it. They really helped me through it.

@FullMoon1917 @DeftBear hi how did the methotrexate go for you both? I had the injection yesterday and haven’t Had any real side effects yet just feel so tired

FullMoon1917 · 13/08/2024 14:09

@liltshake i didnt have any symptoms till i started cramping on day 3. Day 4 i felt really unwell and got admitted as they could see some blood on the scan. Luckily things stablised but the pain and bleeding have continued.

I have been told its all normal but surgery is on the table if i can't take the pain. Hope that hasn't scared you, i think my hcg was quite high when i got my shot and they could see my tube was expelling it all so maybe thats why its been so painful? I have seen a lot of stories of zero pain so i hope thats the case for you! Xx

liltshake · 13/08/2024 14:13

@FullMoon1917 thank you. Sorry you’re going through this it’s awful isn’t it! Can I ask what is the pain like? My stomach is hurting quite a bit but it just feels like constipation, also hurts when i go to the toilet, but not sure if I should be concerned as it literally feels like bad constipation!

FullMoon1917 · 13/08/2024 15:15

@liltshake mine initially felt like uterine cramps, i had a miscarriage earlier this year and the pain was a mild version of that. The ones that got me admitted were like a mix of that and insane trapped wind. I do have a history of IBS so i think the methotrexate triggered an attack? Since then its been pinchy and elastic band kind of pain that happens randomly and when i go to the loo its agony unfortunately.

I will say its not been like this all the time but is particularly bad at the moment. I got scanned yesterday (my husband made them) and the clot grew from 4.5cm when i was admitted to 6cm so they said its no wonder its painful now.

Apparently this is all normal though so just need to rest and relax as much as I can. My hcg has gone down from 2200 to 198 though so its clearly working.

@DeftBear its so much worse than they say but lucky it was only a week i guess! My pain was so bad until we got to basically zero so 4 weeks and even then the blood clot was still nearly 8 cm. Next scan next month to see if its got smaller and then i think a hycosy to see if its cleared the tube. Have you had any follow up?
