How do I get a new MATB1? (DWP being horrific)

I'm on a roundabout and slowly hemorrhaging money. Almost maxed out my credit card buying food.. I thought my claim would be sorted by now
I applied for Maternity Allowance two weeks after baby was born. Shes now 2.5 months old.
I would have applied before hand but my work told me I be paid through payroll but that turned out to be wrong. I was 5 days out from being able to claim maternity pay via them and they got mixed up so only told me that just as I had the baby!
I then applied to DWP for maternity allowance. I used the MATB1 my work had accepted and I included the birth certificate.
DWP texted and requested the MATB1 have the actual birth date on it not just the due date so I took it back to midwife who wrote that on it and re sent it.
I have now been signed off from midwives.
The DWP again contacted me saying that they could not accept the MATB1 as midwife had made a mistake and crossed the mistake out but not initialled it.
They said they now want a completely new form.
They told me to go to my gp who would give me a new form. I did that. The gp receptionist said they don't do that and gave me an advice line number. I rang that they said to try the midwives. I cannot get hold of midwives have left many messages, no reply. I went to the midwife clinic. They told me only the midwife who saw me can fill in the form.. I cannot get hold of her. The midwives gave me the advice line number again. They told me to ask at the gp and a doctor could fill it in. The gp receptionist told me this was not the case.
I rang DWP and they just said again that they needed a new one and the GPs will give it. They wouldn't listen to me saying that they won't.

I'm so stressed. I don't know what to do! No one will do this form for me.

Has anyone been in this situation? What did you do?

I'm also worried because this is only backdated 3 months apparently. So I may lose out on money as well as the credit card fees. It's a nightmare.
I don't have it as bad as some because my husband is still working and so he's just covering all the bills and mortgage etc.. but I have 3 children so we needed my income. Have been having to use credit to buy food. Would be OK if only for a short while but I'm very worried this won't resolve.

I was under the impression they did that all online now, but I'm not sure. Everything for me so far has been online, including exemption certificates but I'm not quite at the point of being allowed/needing a MATB so sorry I can't be of help!

Did you see your midwife who did the MatB1 through the GP surgery? If so, ask to speak to the practice manager about the lack of contact and response and their support in resolving.

If it was through the hospital midwives, ask for a senior midwife or the team manager. If they won't help go to PALs at the hospital to ask for advice.

Sounds utterly infuriating OP, poor you. I'd keep on at the local midwifery team and keep calling the advice line too and insist you need a copy. Make a massive nuisance of yourself until they sort it.

I hate confrontation it's so stressful. But yeah it does look like I'm going to have to kick up a fuss. I'm going to try the midwives again in the morning. And if that fails I could try the hospital that's a good idea.
