Royal Mail Lost Mat-B form help

Need some advice - it appears that Royal Mail has lost the Mat-B form and my maternity allowance form as the letter never got delivered and is still in transit after 2 weeks (I paid for first class signed). What should I do? Should I try and get another Mat-B form from the midwives? I do have a photo of the front and back of the form - if I print this out would that be enough to resend the documents?

also I read some people get SMS after receipt - is that meant to happen?

Who is it you’re sending to? Would they accept a copy or would they want an original?
When I sent to my work they asked for a copy only to ensure I kept the original just incase this happened. I sent it special delivery, I didn’t get a message but could track it using the tracking reference.

I am unable to claim maternity leave from my work as I haven’t been there long enough so have sent the forms to the DWP maternity allowance to claim the govt maternity allowance however the documents never arrived!

ill probably try and get a duplicate copy next week but was wondering if it’s needed. Yes my old employer for my first pregnancy only wanted a scanned copy.

I am now in the same position as this and found your thread. What happened for you in the end? Did your original application ever arrive with Royal Mail? Or did you need to get a new MAT-B1 form and submit a new allowance application again?

Hi @MaryHua ,
The midwifes were happy to provide another MAT-B1 form after I explained what happened.

BUT even though Royal Mail hadn’t tracked it as delivered, the original application actually did arrive and was processed in due course (did not receive any sms etc as some people had). I was about to post off my new Mat-B form when I received the DWP confirmation letter in the mail (from the original application) after they had processed it (I think it was about 2-3 weeks from my postal date).

@Pinkypinker oh thank you so much for your reply. That is super helpful.
My midwife has agreed to give me a new certificate so I'm pleased about that, but I'll possibly wait another week to see if I get an update from DWP. There is no reason why it shouldn't have arrived. Perhaps there is a glitch with my tracking number. Fingers crossed!
