Postnatal appointments - health visitor checks

As I am classed as an ‘out of area’ patient at my birth hospital of choice and am seeing a community midwife at my local GP surgery where I live, what happens after baby is born when it comes to having health visitor checks / postnatal appointments? Who will carry these out and how do I go about arranging these? Will it be the hospital that I gave birth out doing these even though I am not in their local area or would I need to contact my local community midwife number and have them come out? Any help would be appreciated as I’m getting myself all worked up about this haha

I’m guessing that the midwives you are with now will do your postnatal appointments. They should also notify the health visitor (local to you).

Just ask the midwife how it works at your next appointment, or before you leave hospital if you’re close to giving birth.

You shouldn’t need to arrange anything yourself.

I gave birth at an out of area hospital. After birth I was referred over to the midwife and health visitor team in my area. It was very smooth and slick. I didn't have to do any chasing or phoning. The hospital did it all for me. So had all my pregnancy midwife appts and gave birth with my chosen hospital and then all aftercare was transferred to my area.
I made it even more complicated because my doctor surgery was also in a different area. But it was absolutely fine!

I'll be doing it again this time round because the chosen hospital is actually closer and one of the best maternity units in the country.

You'll be discharged back to your Community midwifery team initially who will then refer onto their Health Visiting colleagues.
