Self employed maternity allowance issues, DWP lost my MAT B 1!

Hi everyone not sure if this I'd the right place or if anyone will even be able to help really.

I am self employed and applied for my maternity allowance the first week I was able to. I was contacted last week by them to say that my application isn't complete as I need to send them my MAT B 1 form. I 100% sent this in the envelope with the application form. So I was on the phone to them for over an hour - they said they would get another department to rescan the documents and see if its in there, this could take a week.

Yesterday I received a text message to say my post had been rescanned and the MAT B 1 form isn't there. I really don't understand what's happened here as I triple checked and I know it was in the envelope! My midwives have told me they won't give me another one as they're not allowed to, and this is why thwy told me to photocopy it (which I did, but department of work and pensions told me they won't accept that it needs to be the original document).

I am due to go on maternity leave the first week of October and really don't know what to do. The stress of this is going to send me into early labour! I've been up worrying about it all night. I've got a young son too and my husbands wage won't cover everything, so I'm facing having to go straight back to work with a newborn if I can't get this sorted. Has anyone experienced similar or got any advice on what I can do here?

This exact same thing happened to me in my first pregnancy; I know the form was included, someone must have been careless. However I got a second one no problem, they just had to mark it as a duplicate (I think) due to loss. I'd escalate this to your named consultant or midwife team leader and don't let it drop - you need to be paid during your leave and they are currently not facilitating this!

Firstly you won’t go into early labour through stress, stress actually blocks the hormones needed for labour. Secondly, speak to your midwife again and explain they have lost it. They really aren’t supposed to give out another one but if it’s been lost then they might. Otherwise escalate it with dwp
