High hcg but nothing visible on scan?

Hi everyone!

im looking for some advice/reassurance maybe.

i’m 30yo with PCOS. I’ve just done my first round of IVF. We got 3 embryos and transferred two, both good quality.

I’ve been diagnosed with mild to moderate OHSS, and had some bad pain in my side a few days ago. I went to my local Early Pregnancy Problem Clinic this morning and they did a vaginal scan. They couldn’t see anything and were worried about ectopic. They did my HCG and it is 1594 at 4w4d. The consultant said that she would have expected to see an embryo with those numbers however she couldn’t find anything either in the tubes, womb or ovaries etc.

We have been going mad googling trying to find similar situations. Our hope is that the reason the hcg is so high, but nothing can be seen is that both embryos implanted and that they will be twins.

We have a review on Thursday for the OHSS, but we are worried sick that this will be another failed pregnancy or ectopic.

Has anyone had similar experiences with hcg at this level and seen nothing on the scan with potential multiples?

Sorry you going through this it must be hell, I'm not an expert but I think 4+4 is a bit early to see on a scan which is why she's bringing you back for another, really hope everything goes well 💞

Hey, I was in the exact same position as you the beginning of june,although mine was not through ivf, my hcg was near enough exactly the same too!! & they couldn't see anything on the scan & I too was told they'd expect to see something, I was so worried I also made a thread on here, well i had to go back 2 days later to check my hcg was doubling as it should, it was so they booked me in for another scan 2 wks later & that time there was a tiny baby with a heartbeat measuring at 6 +3 wks!! Fast forward I am now 33 wks with a healthy baby girl 💝 I think it's more over the 2,500 mark that you should worrie if nothing is seen, I hope it turns out the same 4 you, wishing you so much luck xxx

You won't see anything on a scan that early.

At 5+1 weeks my HCG was 14,500 and the sac was just about visible but no heartbeat could be seen. I went back at 7+1 and by that point the heartbeat was clear.

Thanks so much for your replies.
We went to see our fertility consultant today and he was happy with the hcg number and reassured us that he didn’t expect to see anything on the scan at this point. It’s just everywhere I read it says they worry about Ectopic when the HCG is above 1500 and nothing is visible on scan so I have myself freaked out. Getting my HCG done again tomorrow.

Agree it's too early to see anything on a scan. The wait is utter, crushing hell though. Keep focussed on the next step, a good next HCG result and then the next scan. Really hoping for you!

Hcg has come back over 3000 so they are happy it is progressing. Fingers crossed 🤞
