Positive test 2 weeks ago - nothing showing on the scan

Hi all,

I wondered if anyone had encountered anything similar.
I had a private ultrasound yesterday, where they could see the thickening of the endometrium and a corpus luteum cyst which is normal for the beginning of pregnancy.
my test was positive 2 weeks ago - 1-2 on vn digital and faint frer. This morning it is on 2-3 with a strong first response. (Lighter control line)
my last period was 22.10 which would put me 8 weeks Friday which I knew was unlikely as I have very irregular, long cycles.
This ultrasound has thrown me and been classed as a pregnancy of unknown location and I have to go to epu next Wednesday for a scan.
she did say she can see no signs of ectopic at current and doesn’t believe it’s that, but can’t rule it out.
But, a positive test 2 weeks ago, surely something would have been seen by now?

thanks in advance!

It's way too early to see anything on the scan. Try not to worry.

Jjieo · 15/12/2022 06:27

@plantsareglorious thank you, I think I’m struggling because the sonographer made me feel she should have seen something by now with a positive test 2 weeks ago.
I had the scan for dating as my cycles can be 35-55 days.

Theoretically, you could only be 4 weeks pregnant. In which case a scan would show nothing - usually the thickening you mentioned and perhaps a little black space. I'm really surprised EPU will see you in a week - at 5 weeks you'd also just be expecting a (larger) black space and maybe a start of a yolk sac

At this point it's likely you just ovulated late given you have long cycles!

Jjieo · 15/12/2022 06:31

@IWillBeWaxingAnOwl I think, because technically I’m meant to be further they want to check as no pregnancy could be seen in my uterus so they need to rule out ectopic. She said it’s 1 of 3 things:

  • too early (but I would have thought with 2 weeks positive test it would show something?)
  • ectopic
  • or miscarriage

she said they want to do the scan at epu to see if they can view the pregnancy, and if not they will need to do bloods. Hcg is definitely increasing, can tell by the frer! Just feel very confused, and didn’t expect this.

I've had this twice now - NHS are always a bit baffled by long cycles (so late ovulation, so didn't conceive when they thought I did). Both times I've had a few scans, as the first one they were concerned was behind time wise (they weren't, as I knew when I ovulated). One pregnancy sadly ended but it was growing fine for ovulation dates until 11 weeks, one is still current at 18 weeks today.

Basically I hope that your scan next week is consistent with 5 weeks, that this is the start of something exciting for you, and rules out ectopic 😊.

plantsareglorious · 15/12/2022 06:35

It's just too early that's all, take some deep breaths all should be well, I really don't think there's anything here to indicate otherwise.

Jjieo · 15/12/2022 06:38

Thank you @IWillBeWaxingAnOwl @plantsareglorious it wasn’t a planned pregnancy, I have 3 children and never had this happen so it’s made me wobble!
you’ve given me me reassurance and will keep you updated either way! Thank you x

It’s far too early to see anything.
Was there sonographer an nhs sonographer? Or an unqualified person working at these baby scanning places?

elevenplusdilemma · 15/12/2022 06:58

Way too early - from the fact that initially the tests said 1-2 weeks and have now progressed to 2-3 weeks, it would appear that you're between 4-5 weeks now.
If you're lucky, you might just about see something on a scan at 6 weeks (transvaginal), but could take longer.

somethingluscious · 15/12/2022 07:17

Do you know when do you normally or did you ovulate?' My EPU won't see me until 8 weeks after start or last menstrual period. So I tend to end up with scans at just over 6 weeks, which is still pretty early. That's with me ovulating about CD25 ish. I calculate ov as 2 weeks & add from there to get likely gestational stage.

Jjieo · 15/12/2022 07:28

@Rainingoutthere this was an nhs sonagrapher for dating who didn’t offer much in terms of is this normal, all she said was well it’s 2 weeks since a positive test I would have expected to have seen something so I’m referring you to epu.
my lines was faint when I tested, I only tested because I’d been in an awful mood 🙈

@somethingluscious this is when my flo app says I ovulated, I wish I knew when we had dtd, but it was in the window of ovulation as I looked at the app and said to my oh I didn’t realise I was potentially fertile! We only did it twice last month as I had a urine infection before.
I had a positive test 30th nov x

Positive test 2 weeks ago - nothing showing on the scan

I had a private scan at about 5/6 weeks and they couldn't see much at all; told me to come back in about ten days and there it was!

Good luck.

Jjieo · 15/12/2022 07:33

@elevenplusdilemma so I initially thought I would be the 6 week mark probable, midwife was going to get me to do all the booking and bloods and I said to her I’m certain I’m not as far as my lmp would put me.
I also would have expected the clear blue digital to have change to 3+ now but I guess I’m judging by previous pregnancy perhaps!

OP posts:
somethingluscious · 15/12/2022 07:46

The clearblie won't always change to 3 weeks. It's hCG threshold is up near 10000. I think that makes you just over 5 weeks so could still be too early. I wouldn't really want a scan before 6 weeks for tha lt reason.

Jjieo · 15/12/2022 07:51

@somethingluscious I assumed by my app I was about 6 weeks because usually I would be the same. It had to be dated but I was shocked when nothing could be seen 🙈

I also have a tilted uterus, I wonder if that restricts view.

OP posts:
TheWeeLittleDonkey · 16/12/2022 07:08

Hi OP. A tilted uterus does impair the view.

I’ve had recurrent miscarriages so I get scanned at EPU every two weeks. By LMP I was 6w but I know I ovulate late. They couldn’t find anything on the scan other than thickened lining. I had to go back the following day as my HCG levels were so high they knew they should be able to
see something and the consultant found the pregnancy where it should be located.

I had a private scan earlier this week, 12 days after the scan they saw the gestational sac and yoke sac, and at that scan they saw the tiny embryo with a heartbeat.

Jjieo · 18/12/2022 20:28

Just to update you, I got asked to have bloods on Friday by epu, Friday my hcg was 1069 and today around 45 hours difference it is at 1828. She’s told me there’s still a chance it is ectopic and she would have expected it to double. She’s kept the scan for Wednesday as she has said with this hcg level now something should definitely show.

Jjieo · 20/12/2022 07:26

Just to update this, for anyone that comes across this thread. This is an ectopic pregnancy I’m waiting to have surgery now.
intuition can be good sometimes x

Oh no. I’m so sorry to hear this.
I hope they are able to treat and you haven’t ruptured yet.
