Severe Cramping and Empty Gestational Sac - Advice Please!

Hi,
So I’m around 5 weeks + 5 days pregnant and for the last week I have been experiencing mild to moderate ‘period like’ pain across my lower abdomen which I presumed was normal due to my uterus stretching. However, yesterday while at work I had severe cramping which lead me to feeling faint, I had blurred vision, cold sweats and almost passed out. I ended up in the EPU where they did a transvaginal ultrasound to check for ectopic pregnancy. The midwife said that she’s certain it’s not ectopic, however she could see the gestational sac, but no yolk or fetal pole was present. She said usually at almost 6 weeks she would expect to see at least something by now and to go back in 10 days to see if there are any changes.
All of this had got me petrified that I may be miscarrying. I’ve been obsessively reading about blighted ovum’s and MC horror stories. This is my first pregnancy so I’m completely new to all of this. I’ve also been reading that it can be common to not see anything on ultrasound this early, but that alongside the severe pain and almost passing out has got me really worried.
It might be worth mentioning (sorry tmi) that I suffer from IBS and every time I get these period like cramps it’s usually followed by getting the urge to go to the loo. Not sure if there is a correlation between IBS and uterine-like cramping?
I know there’s no definite answer at the minute and I’m just going to have to go through this long dreaded wait until next Saturday, but was hoping that someone may have had a similar experience?

I don't have anything comforting to say, but I wanted to post to say that you're not alone. I am 6+2 and I went to the hospital at 5+9 after cramping and bleeding. She saw the sac so ruled out ectopic but there wasn't much in the sac and she thought I had my dates wrong (I don't - I know exactly when I ovulated). Like you, I have to go back in 10 days. I'm still cramping and spotting. She said to me that some women have this and are fine and some have this and miscarry.

The other day I had severe, severe cramps for about a minute or two that I at first thought were uterine. My first thought was that the miscarriage was coming, but then I realised that I needed the toilet and it was diarrhea. I've never had it like that before with abdominal cramps.

I was scanned at 5+5 and all they could see was a gestational sac which they wouldn't even confirm - said 'most likely'. Went back 2 weeks later and baby there with a HB measuring 7+5! Am 18+2 now and so far so good. I've also had constant cramping the whole way through! The EPU didn't seem concerned at all about only seeing a sac and said it was perfectly normal before 6 weeks.

Thank you so much for both of your responses ladies.
Unfortunately my cramps have became far more frequent. I have since spoken with another midwife who pulled up my scan report. What was not mentioned at the scan was that my gestational sac is also irregular in shape which could also be a sign of a miscarriage. It just seems like the odds are stacked against me, with the empty irregular sac and the intense cramping. I’m just confused about why I’ve not had a single bit of bleeding yet and how long this pain can continue until bleeding eventually starts.
My second scan had now been moved forward to Monday with a different midwife for a second opinion so will wait and see if anything different shows up.

@Rebecca2494 I think there is debate around whether irregular shaped sacs are an indicator of miscarriage: pubmed.ncbi.nlm.nih.gov/21193702/

Just a quick update- I had my scan today and it's good news! Turns out my dates were wrong due to me ovulating a week late, it's essentially set me back one week.
Everything is looking as it should be at 5 weeks +5. Gestational sac is now a regular shape and tripled in size since last week, yolk sac and fetal pole are now present and we could even see a little heart beat!
The midwife didn't seem overly concerned about my cramps and said that it's probably down to my uterus stretching alongside my IBS. Overall a massive relief!
