FW8 form for free prescriptions/dental treatment - who needs to sign??

I've had my form since my booking appt at 8 weeks.

My MW signed it but there is also space for my GP to sign and a space for a GP stamp...

I've heard that just the MW signing is ok - anyone know if I need to get my GP to sign it too? Confused

I needed the stamp from GP surgery and signature from MW.

My MW signed it at routine appointment and got it stamped at reception!

I was given a blank form from my midwife at my first appointment at 6 weeks, she had a stockpile of presigned and stamped forms! Just got my card through, GP didn't have to do a thing :)

I had a signature from mw then took not to gp and they stamped it a sent it off. I had the card within 10 days
