4 year old toilet training going backwards

Help! DS was 4 this week, and toilet training still not going well. First took himself off to the potty just after 2nd birthday, since then it's been 1 step fwd, 2 back. Weeks or months in pants, then back in pull ups. Finally, after 3rd birthday went for pants permanently. We have good days, or even a week, then he just can't be bothered. Easter this year we were getting about 1 'accident' a week, but have now regressed again, and have had 15 this week. It's deliberate and laziness, I'm sure. Have tried sticker charts (useless), immediate rewards (limited success),long term rewards (same), threats, removal of toys, ignoring it, leaving him in wet pants, and last weekend DH resorted to putting him in nappy after not doing any wees on toilet all day (he cried for and hour, then was dry for 3 days after). GP keeps telling me it's still within 'normal' range of toilet training and he'll grow out of it, but am at end of tether, especially as he starts school in Sept. DD (5.5) got it in a matter of weeks at 2.1. What can I do??????
