3.5 year old toilet training regression...help please

We toilet trained our daughter in October just before she turned three. Took a few weeks but she finally got it and hadn't had an accident since November, bar the very odd one where we were too far from the loo and just didnt get there in time. She has been reliably telling us since about January. Until two weeks ago. Shes suddenly having multiple accidents a day...on Saturday she had 4 in one day despite being at my in laws and being close to the toilet and us at all time. I ran out of clean clothes and had to put her in one of her old nappies mu in laws still had. She does tell us sometimes but doesn't even mention it if she has an accident. After a week I took her to the doctor as she also said it hurt when she weed and tested a very faint positive for uti (wasnt even sure it was postivice) so had a few days of antibiotics as a precaution which we finished on Friday. But its just gotten worse.
We use a pull ups overnight though shes dry most nights and when shes not its the odd wee but this morning it was completely full and saturated.
I dont know what to do. We are trying to stay calm and just remind her lots but we are at the end of our rope. Help any advice??

She did test positive for a UTI so I’d start be going back to the doctor in case it’s gotten worse and hasn’t responded to the antibiotics as it should. Also is she drinking more in this hot weather? You may just need to pro actively take her on a schedule for a bit then ease off once the accidents have stopped.
