Toddler suddenly started holding in poops

Please help! My 3 year old is potty trained for a year now but has always refused to poop unless wearing a nappy. I’ve spoken with GP and HV about this and they said it’s very common.
however this past week he has suddenly stopped pooping. He is in agony but will hold it in.
We have tried everything, bribes, gentle explanations, offering toilet/potty/nappy. He has been having lactulose for 5 days with no help.
Has anyone else experienced this? Any tips/advice?

No advice but this just popped up and couldn't ignore something - lactalouse might not be enough, it just softens the stool - you will need another laxative too (Senna - these might be otc but do ask). Having said all that, I'm not sure that will be the right thing to do for this situation. (Sorry if unhelpful!)
