Help! Poo withholding problems

So my daughter (just turned 2) decided that she no longer wanted to wear nappies. She wees on the potty with no issues, and lets us know when she needs to go. However, pooing is another story altogether! It's been almost 4 weeks now and she is holding in her poo for days at a time. She is clearly uncomfortable, in pain and very sad at holding it in which is really heartbreaking. We have tried all the usual things but we're just not making progress:
Lots of potty time, good high fibre diet, very relaxed about it and lots of praise, rewards, blowing bubbles, and she's now on daily lactulose to reduce constipation. She will also not poo in a nappy. Has anyone has been through this and have any tips to share? Or even reassurance that it is a phase? I'm so worried that this will have long term consequences!

The ERIC website has a page on withholding pooh. Have you tried bubble or balloon blowing....or party blowers.

Thank you.. I've been using advice from the ERIC website, they give loads of amazing tips. We have a box of party whistles, bubbles and little windmill toys by the potty! Nothing seems to be solving the problem yet, I guess I just need to give it more time
