How to rule out medical issues

Hello,

I have a non toilet trained 5 year old. He has additional needs. He can't really communicate pain properly.

He is petrified of the toilet/potty. He's never done anything on the toilet/potty.

I'm not convinced there isn't a medical issue or something causing his pain. As he isn't communicating being in pain GP won't even see him.

He recently broke his collar bone and complained of pain once the entire time so it would make sense for there to be an underlying issue that isn't being communicated.

Are there steps to rule out anything obvious?

Constipated - bristol stool chart and look for overflow
Worms - you can just treat everyone at home and again two weeks apart
UTI - get urine tested by the GP
