Regression?

My DD is 3 and only fairly recently toilet trained (August).  She does well most of the time but seems to have a day or 2 a week where she has a terrible day and has maybe 3 accidents.  I've thought about it and tried to see if there is a pattern but there doesn’t seem to be?  She has also started a new nursery where she is having at least one accident per day (she’s there 3 days a week).  It feels like we’ve taken a step back and I’m just not sure what to do.  Any help/advice appreciated.
