Potty training woes .. Any advice?

My just 3 year old girl came out of nappies just over 5 weeks ago. She quickly got the idea of potty training and was dry at home with a week and a half, she no longer asked, just went and then told me when she needed help wiping etc.
She was going for two days to nursery, in the last month she has not managed a dry nursery day. Having several accidents a day with only maybe one or two successful trips to the toilet. I understand that nursery is busy and it can distract them but nothing can distract her long enough at home. She's eating, she goes. She's watching a film, she goes. She's playing in the garden, she goes. Playing with friends, she goes.

Although I thought this was a concern, she was doing so well everywhere else that I just thought she would need time.

She started pre-school last week. Every morning & has now dropped to just 4 hours of nursery a week (one afternoon)

She now has accidents everyday, at least once. In the 4 hours at nursery last week she has 3! I have had trouble discussing it at the nursery & i'm going to insist it is discussed when i pick her up today.
The thing that is concerning me is now she no longer lets me know when she has had an accident & then when i see she is wet she refuses that it is wee. She's having them at home, in the car & all the places she seemed to have mastered.
I thought it was maybe my reaction to an accident. I started out with plenty of patience & after thinking she got it I have probably changed to frustration when there is an accident. I have started to revert back to praise when she's successful & i'm hoping she will pick up from where she was doing so well.

My question is, does this ring true for anyone else? was there anything you did that helped improve it? Does it ever end?!?! Hmm
