Toilet inserts for compact toilets

Do all inserts/rings you place on the toilet itself fit all toilets? We are putting a compact toilet (slightly smaller than your average sized toilet) in our downstairs bathroom and are thinking ahead as to whether an insert/ring will fit?

Do you really need an insert? Smaller toilet = smaller hole. Neither of mine have ever used one.
