3yr 10 months - advice please

My ds is 4 end of November.

I have been potty training him using pull ups for nearly a year.

For the past 3 weeks we put him in pants in the day. He knows what, where and why to do it on potty or in toilet and is quite happy to do it.

BUT we are having accidents all day long, he just does not seem to know he wants to go until he goes, sometimes we make it sometimes we dont.

My eldest ds has kidney problems which means he does not get the urge until he needs to go - then needs to run.

I had a scan done on little ds and it showed enlarged kidney, but because Doctor not given them my other ds notes they said to re scan in a year.

Do I put him back in nappies? it is very difficuly doing a complete clothes change when we are out due to disabilities I have.

Also, today pre-school suggested a doctors visit - I have an appointment in a while.

I would give it a little bit longer with the pants.
does he sometimes manages to go to the loo?
maybe you have to lead him to the loo every half hour or so and give reward for doing anything in there.

yes I take him every 30-40 minutes he always does something.

He gets stickers which he loves and when he started wearing pants he got a pirate ship.

When he wets it just seems a complete shock and he gets upset
