5 year old reverted to pooing in pants...help!

Not sure if this is the place to post this, but for a few months now, one of my twins has been wetting herself on a fairly regular basis at home and school, which I put down to a lack of concentration, and being a bit lazy running upstairs. However, she has now been pooing in her pants at school and at home or while we are out. It is so frustrating, and I am starting to lose my temper with her every time, which I really don't want to do. I have tried star charts, bribery, threats (ie wearing a nappy) and nothing seems to work. The staff at school are getting a bit fed up with it, and when I ask her why she does it, she says she didn't know it was coming. I want to take her to the GP, do you think it will do any good, or will they brush me off? We are now expecting dc3 and all this started before they knew about this, but I want it to stop before he/she arrives. If anyone has any ideas, I would be very grateful to hear them!

Definitely take her to the GP

She could be constipated

Maybe, but she does at least one good sized poo every day, sometimes hard (apparently todays was), and sometimes very soft. She has a good varied diet, with plenty of fibre and fruit and veg.....
