Hi I have permission from MN powers that be and have emailed them to let them know it's being posted today. Here is what she has written about it.

I started to fill it out but then stopped as I couldn’t put my qualifications in as it only allows either stem or arts and I have both.🤷‍♂️

She's looking at it now to see if she can change it to be able to pick multiple qualifications. Have a look later if you get a chance to see if it changes if you can.

I completed it for 3 cats and various dogs but there was no option for gerbil so I said they were hamsters (sorry gerbils!)

Yes she is. I really like going there for pick ups and drop offs, it's a nice town.

I used to drive over from Kent and having never been to Wales before I was truly stunned by the beauty of the landscape

One thing I noticed about it was that you couldn’t tick more than one option in the section asking what antibiotics had been prescribed for- so couldn’t tick, for instance, both respiratory infection & ear infection.

There wasn't any don't know options for the questions so it was hard to answer. Also there was a load of statements about the thing that you can't agree with if you've never heard of it, but it won't let you not tick any for that question. So it's fairly poorly designed. I did it though.

Agree, I'm old and have had many pets over the years and they've been given antibiotics for all sorts of reasons.

Also, some of the questions give you Yes/No/Prefer not to say but there's no option for 'I don't know'.

This isn’t possible to complete, there are questions that need N/A as an option.

The question about Bactiophages (?) - where is the option for “don’t know”?
