Advice needed- Cat third eyelids

My neighbours just told me their cat was poorly, and that his 3rd eyelids were showing.. from experience i thought this means that there is something wrong? I told them that they probably should see the vet as there must be a problem, they told me they cannot afford it (benefits and spending money inappropriately on stupid things) i got quite angry and told them it's very important they go as soon as possible. Am i correct with what i'm saying? Is there a serious problem?

I feel so bad, i might offer to take him myself ☹️

Yes especially if he is lethargic and sitting hunched up

My cat had this at the beginning of the year, the vet told me it was a cat version of a viral rash.
It usually means they are a bit under the weather and sometimes something else will come out - in our case our cat ended up with horrific diarrhoea for a week.
Ideally the cat needs seeing by a vet.
