It's quite a long survey but there is a 5 minute option from Sex Matters here :

Comment on the schools guidance – even if you only have five minutes - Sex Matters (sex-matters.org)

Please complete this if you don't want your child told to be told that their female friend or classmate is a boy or their male class mate is a girl.

Please also complete it if you don't want to run the risk that your child themselves starts to wonder if their hobbies, personality and taste in clothes might mean they might actually be the opposite sex.
