Is it possible to change my ds's surname?

Was not sure where to post this question, but I hope someone can help me.

My ds was given both my and my xh's surname to make it a double barrelled surname, which I also took when we were married. Now we're divorced (hooorrayyyy) I have changed all my bank accounts etc back by simply lopping his name off the end(wanted to lop off a lot more I can tell ya).

Today, when I picked up a prescription, the doctor had written it with my ds's name on but just the one name not the two usual for him, and said she thought that I had changed it. The question is, can I actually do this without my xh's consent or knowledge?

don't think so as i think your dh would be construed as having parental responsibility for the purpose of this.
