DD hates wet and dirty nappies and can mount the toilet - just wont go!

Please help.
My DD will be 3 at the end of September and is still in nappies. However as soon as they are wet or dirty she wips them off and asks for a clean one, or knickers. Once in knickers she stays dry until asking for a new nappy or the bath (yep she pee's like a fountain in there).
We've had one small wee on the toilet about a week ago - we've been at it since July. But I think it was more luck than judgement as minutes later we had a puddle. She got loads of praise for having a wee in the toilet and a sticker on her chart and a special sweety. Now I am worried she's holding herself to the point of pain - in which case she can carry one stripping of her nappies, I dont want her in pain. But she knows how to use the toilet and wipe herself as she does this all the time, without actually doing anything, she loves knickers and hates soiled nappies. All signs say she ready to go, but she just wont let go.....any ideas?

I'm not very good at this whole potty training thing but am bumping this so hopefully someone who is will come along.
