Try scrubbing his face whilst he writhes around violently trying to avoid the cloth. This technique (previously untested or even thought of) proved particularly effective with ds1's first tooth to come out (though did result in his unwittingly swallowing it and we didn't realise anything had happened until the gum bled a bit ).

go for it - I pulled ds1's first one out in the middle of a cafe because he couldn't eat his meal properly.

grab it in a tissue and yank ( you need the tissue to get a firm grip)
