I once got in touch with Startrite shoes as I was idly thinking about getting shoes from them by mail order for dd (some shop in Norwich sends them). They sent me a measuring 'kit' - basically a piece of card with all the sizes like you would get on a shoe shop measuring thing, but better than nothing.

Vertbaudet used to do a shoe measure but think it was european sizes. Similarly Naturino enclose a paper one in their shoe boxes but neither of these help with width.
