15 hours free for 3-4 year olds

Cant find any info at all really so I thought the best place to ask will be mumsnet.

My DD is turning 3 next month so will be entitled to 15 hours free childcare from after Christmas. She currently attends a nursery 2 mornings a week which we pay for.
My question is do I need to do anything to get her 15 hours free? Apply for it or fill out a form or anything? All I can find is info about 30 hours free and 15 hours free for 2 year olds (neither of which we are entitled too)

Is she staying put or going to a different nursery. DC1 is in private nursery and the nursery gave us the forms for the funding.

My DD is starting playgroup in September for 15hrs free p/wk, all I had to do was fill out a details form they gave me and take in her birth certificate so they could take a photocopy

They should do it for you, you will have to sign something to agree to them claiming the funding. Do they definitely offer funded places? Not all settings do. I would just ask them.
