How do I approach another mum at nursery and ask for her name? We know each other as so and so's mummy so far. I'm quite shy and don't have many friends :(

You just say 'oh by the way, I'm Pity' then she will say her name, then you'll prob joke about how once you have kids you just become so and so's mum.

I'm a bit shy too but I've made loads of friends since being a mum. You can always speak about DC so there isn't the awkwardness about what to talk about. Hope it is the same for you.
