Nursery leaving nappy on all day

Hi,
Can someone advice pls? My son is 2 1/2 and still in nappies. He does 3 full days in a lovely nursery with the most wonderful caring staff. Now last october he come home once with a nappy that I recognised being the one he went out with in the morning so I mentioned it to the staff, it was slightly embarrassing for them and it look like an oversight. Yesterday however it happened again. I overall really like the nursery especially as he has changed and is much happier now. I am not sure what to do and if to mention it again to his key worker, or go to the manager ? It's such a well run place and I can't believe this is happened again...
What is the best way forward?

That's terrible, definitely request meeting with the manager and ask how she will ensure it does not happen again. I thought 4 hours max in nappy unless dirty was the policy?

Definitely mention it. My DD is the same age and at nursery full time. Her daily diary sheet always shows 3-4 nappy changes. I'd be livid if she'd been left in the same one all day. It would make me question staffing levels and what else they might be overlooking.

As a nursery nurse and a manager of a nursery I would want a parent to mention that a nappy had not been changed all day. It should not happen especially twice and there should be a changing sheet for all the children that states who has changed them at what time and if it was wet or soiled. You can ask for one to be put in place. If you have a meeting with the manager you can explain it isn't the first time it's happened and you are happy with everything else. It really shouldn't antagonise them. I hope it goes well.
