Right to choose ASD for me - what do I ask my GP?

I have an appointment with my GP to discuss my mental health and some struggles I've been having forever but that have got worse over the last couple of years.

My sister has recently been formally diagnosed with ADHD and our dad is almost certainly autistic. I have a long list to discuss. I feel I need some sort of psychiatric assessment and it'll help me understand all these things that are 'wrong' with me.

I'm aware I have the 'right to choose' but what does this mean in practise? Do I need to find a clinic myself that I ask them to refer me to? How do I do this?

Any general pointers are much appreciated
