Hi everyone

I've been wanting to be referred by my doctor for a long time. However, I've never been able to get myself together enough to write down behaviours and so on. I came across a diagnostic form that I've printed off and filled in, and which I'm going to use as the basis of a conversation with my doctor to get a referral. I know they take years, that isn't the presenting issue for me right now. I just want to get something in my medical notes, and will think about what to do next once I've spoken to doctor.

Scroll right down to the ADHD part. When you click on the link for Adult ADHD TEST a PDF form will download. I've found this form helpful as I can't do open questions, so if asked about how ADHD presents I wouldn't be able to answer. Suffice to say, there are very few boxes on this form that I haven't ticked!

They have also helped me remember things, and I've been able to think of other examples because of the clues in the form about what these examples are!

It seems to me a credible form used by experts, so believe it to be of help.

Thanks for sharing, I’ve got an appointment with my GP this week and the chart sounds very useful. Could you paste a link to the actual form though? I can’t find it on the website

Have you tried setting alarms on your phone? I use alarms or calendar appointments with alerts for absolutely everything…. (Including reminding myself to phone the doctor or go to the shops…)
And yes I often silence the alarm and then forget again, but I just keep going through the cycle until I eventually get the thing done 😂
