Looking for experience or info on combined meds for ADHD?

I'm currently going through titration and am on my 3rd month of meds. I'm now up to 50mg of Elvanse and it's been great but because I'm a shift worker, I'm having some issues on my working nights.

I work a few night shifts a week but on my working days I still need to be functional in the day for the dc.

Up until this week, I've been taking 30mg at 7am, then another after a little pre work nap at 10pm to get me through my shift. The Elvanse works really well for giving me energy, motivation and focus but doesn't seem to stop the 'inner narrator' and overlapping thoughts - is that normal?

At 50mg this seems like a a lot to be taking in 24 hours, it also means that I can't take one when I get up the next afternoon, (otherwise I'd run out too quickly) making the day after a complete write off.

I've tried splitting the capsule and taking half and half but this didn't work for me at all as the lower dose had no effect and I was useless all day and have needed to mainline caffeine.

I'm still under a private psych so keen to get everything smoothed out before I attempt to transfer to gp care. Does anyone use combined medications to ensure you can get through long days?

or is there something else I should be looking at to 'top up' my meds, like nootropics/something holistic etc?

I take 50 mg Elvanse every morning and also have a prescription for 5mg Amfexa (basically the same drug as Elvanse but fast release) that I can take when I feel I need them. Unfortunately I haven't been able to get hold of Amfexa on the NHS, so haven't been able to do this for almost a year🙃

If you're titrating privately it might be an option to try? I have a review appointment with my private psych this month and hopefully he will give me a prescription for Amfexa that will last me a while.
