ADHD assessment questionnaires

I've got an assessment booked in a few weeks. Who did you get to fill in the evidence questionnaire for you?

My parents could, I don't spent an awful lot of time with them these days though but of course they knew me as a child. I could also ask a friend.

My boss. They have ADHD and suggested to me I may have it too and they encouraged me to look into it in the first place.

I think someone who lives with you and observes your behaviour could be good but you could ask friend who has known you and spent time with you over a year or more.
