This survey is flawed. There are no N/A options. I do not have a DH/DP and my ds has never had/known a dad. So questions such as "How does this differ from how your DCs would describe their Dad?", "I want my DH/DP to help out more and equally share parenting responsibilities with me" or "If at all, do you have a different parenting style than your partner?" which do not have a N/A option can not be answered for families such as ours.
