Have asked for CTC claim form 3 times. 3 wks later -nothing!

I'm going out of my mind! Last claim was stopped 3 weeks ago when I changed my circs and they said they'd send me a new form to complete. Have chased them up 3 times in 3 weeks but still no form. What's going on!? Can't claim my surestart grant (33 weeks preg) either until this is sorted so I'm completely skint! Can't even claim income support because I am still classed as a student until July (just completed degree).

You can claim online here but it's down at the moment which doesn't help. There are some other points of contact listed, however... HMRC Enquiry Centre, Jobcentre Plus Office... which you could try. And I'd be tempted to increase the call rate from one a week to two or three a day. Stay on the line until you get commitments, ask to speak to supervisors.... make a nuisance of yourself.
