Underpaid state pension - letter from DWP

Has anyone been through this/have any knowledge?

I have received a letter from the DWP stating my Grandads state pension was underpaid from the time my Nan died until he died and they wish to pay it back.

Further digging I have found this has been a big case with the DWP with thousands underpaid through one circumstance or another.

My question is - I assume from this he should have received a widows pension from Nans state pension?

I just feel a bit gutted I didn't know this at the time to make sure he got it when he was relying on me to help him - like I've let him down in some way? Although the rational side of me says the DWP should have known this when they were advised Nan had died!
