Working tax credits on smp

So today I received a claim form for working tax credits. I'm due to start maternity leave for 39 weeks on the 14th July. But it only asks me for last year tax year income which is my full wage, so how can they consider my application if they don't know that I'm going on maternity leave?

Do I send a note with the application to inform them of this?

Complete it with last year's details, then once you have received an award notice you need to call them (or write) to tell them your expected income for this tax year. You can also knock £100 per week off your income for each week you are receiving SMP.
