I've seen one called "thinking of working for yourself" with all the relevant detachable forms inside for getting registered.

If you go to your local tax office they should have them available in reception for you to take away.

hmm, if i knew where my local tax office was, yes maybe :)

It'll be in the 'Phone book under Inland Revenue, believe me big brother is everywhere.
