PPI, spoke to bank, what now?

I rang my bank about potentially claiming back PPI on loans that I had a long time ago with my ex. They said they could see two things on their system and that they would start the complaints procedure rolling.

I've received a form from them that I have to fill in, but as it's a long time ago I have no paperwork and don't even know what the account numbers relate to. I also have a dreadful memory so can't answer half the questions!

So what do I need to do to get the details that I need from the bank? Is it a SAR? Sorry I know very little about the process but I am anxious to get this sorted asap.

Thanks in advance if you have any advice.
