Incomplete Miscarriage- Retained products of conception too small for surgical procedure.

I started to bleed at 10w3d, had a scan and they said I had a missed miscarriage and the pregnancy had probably stopped developing at about 5-6weeks. I had to allow 2 weeks in case the pregnancy was developing slowly or id got my dates wrong (i knew this wasnt the case but i understand they have to be sure). I then had cramping and heavy bleeding over the week following which then slowed to a stop so I assumed my miscardiage was complete. 2 weeks after the initial scan i had another one and was told there was a small amount of RPOC left so opted for the medical management (my other option was to wait another week and see if it completed naturally, i wasnt offered the D&C) however a week later and nothing had happened and the medication hadn't worked. I was rescanned today and told there has been no change but that the retained product is about 1.7 cm Maximum. They have said that its too risky due to the small size to do the surgical procedure/ D&C and said I just have to wait and hope my hormone levels drop to allow me to have a period and hope it passes with that.

I'm really struggling with it all being unknown and just waiting as this may not work and I could be in the same position in 4 weeks when we will review things. Does anyone have any experience of similar situations? Did your period return and pass the remaining tissue? How long did this take?

Thank you in advance for any advice or information and I imagine if you've read this you are going through/have been through somthing similar so I am so sorry for your loss and hope you are doing OK.

I went through a similar experience about 3 years ago. MMC found at 12 weeks scan, no signs of natural miscarriage so was given medical management, it partially worked and was given a second dose 1 week later but this wasn’t fully successful either. I was advised to wait for another period and return for a scan. I did have a period/ further bleeding but scans showed rpoc were still there. They booked me in for a D&c , arrived for the procedure, was gowned up and everything and the surgeon arrived and rescanned me and said that the retained products were too small and he wouldn’t do it because there would be a high risk of damaging my uterine wall. Sent home to wait for another period, I bled for several weeks on and off and went back for another scan (at my request!). products still remained! I was then offered a different procedure where they removed the retained products whilst I was awake using local anaesthetic and a laser ( I think the procedure is usually carried out to remove polyps) the procedure wasn’t very painful just uncomfortable but it was a good job I had it done because they retained products had calcified! The whole process from miscarriage to finally getting it removed was 3 months. I didn’t have any further complications and conceived no problem 2 months later. I would ensure you do follow up for scans because I had to be persistent it getting them to check me

@montimama thank you for replying. I'm sorry you went through thay as well but glad to hear that you conceived with not problem afterwards.

I'll keep pushing for check ups and hope it gets sorted.

Hi @Heartbrokenandconfused22, im sorry to hear you’re going through all this.

I was in a similar situation earlier this year, found out I had a mmc at almost 12 weeks the start of May, the fetus stopped growing at 7 weeks and 5 days with no signs of miscarriage until I went for the scan. I was only offered medical management at the time which I took.

Fast forward 5 weeks later and I was still testing positive with pregnancy tests and had not had a period yet. I went back in to the EPU, they took bloods to check HCG levels and confirmed that while the levels were decreasing, hcg was still present. I went for a scan after those tests but it didn’t show anything. I was offered a second round of misoprostal to try get rid of any tissue which might have been hidden in the endometrial lining. I took the tablets but nothing happened.

I went back for another scan in July as was still testing positive and had not yet had a period. This time they noticed some retained tissue approx 2cm, and was offered the D&C or an MVA. I choose the MVA as it was a quick procedure and performed with local anesthesic so no down time and less risks. Following the MVA towed the end of July I finally had a negative pregnancy test and my cycles returned to normal 2 months ago! Still ttc but glad that’s behind me and hoping you’ll be able to put it behind you soon too, best of luck xx

@Heartbrokenandconfused22 how did u get on did the rpoc come out naturally on your period?

Hi @Tclm I did. I had a period around 6 weeks after the initial bleed. It was heavier than normal and I passed the RPOC. We have gone on to conceive again and I'm due to give birth to a baby boy in 2 weeks. 💙

Aw congratulations can i ask did you constantly bleed for them 6 weeks?

Heartbrokenandconfused22 · 09/09/2023 14:29

Thank you.
No after the initial bleed when I was miscarrying which probably only last a week/10 days I didn't have anything other than a tiny bit of spotting until I then had my period about 6 weeks after it stopped. This came on quite suddenly and we heavy straight away X
