Can no longer climax, will HRT help?

I’m post menopausal and recently had the minera coil removed so all hormones gone. I can no longer climax at all, even with a vibrator. Would HRT help restore this?

Yes HRT can help, but especially if using topical oestrogen. That can help significantly, if also applied to the clitoris and surrounding area when using it.

Ovesse has been life changing in the 12 months that I’ve been applying it. You can buy it OTC in boots if you struggle to get to see the dr. It’s taken me several months of buying it myself before managing to get it on prescription, even though I’ve been on HRT patches for several years.

What helped, massively, for me was testosterone.

Thank you for the advice, it’s been quite a shock losing this.

I agree. Oestrogen will definitely help but testosterone will help increase clitoral blood flow so you might need both.

Local oestrogen cream. Ask your gp - loss of sensation is part of vaginal atrophy and they should prescribe it.

Have you tried a clitoral suction toy instead of a traditional vibe? The Satisfyer (ridiculous name), the Lemon, or the Womaniser (don’t get me started…)

Good grief, this is a whole new world to me. Without going into too much detail, an industrial stimulation vibrator would struggle to get anything going.

I went to the gp about this same problem last week. He didn't give me testosterone, instead oestrogen and progesterone and told me it would take 6 months to work? I've yet to start it as waiting for the pharmacy to do the prescription

To be frank, it probably would struggle. The clitoral hood can fuse; the clitoris can shrink and be all but undetectable.

Local topical oestrogen cream can plump things back up. As pp said, testosterone can improve libido and desire. You can only get testosterone if your oestrogen levels are optimal (as the body converts T to O if that’s lacking).

Talk to the GP. You don’t have to live like this.

Excuse my ignorance, but is that different to HRT?

In my case, I didn’t have to have a blood test to get testosterone prescribed.

That’s not saying it was straightforward as my GP didn’t want to prescribe it, so he referred me to a gynaecologist. At completely unnecessary expense to the NHS, I went to see a gynaecologist for a 5 minute consultation. I said ‘I’d like testosterone please because my libido‘s fallen off a cliff’ and he wrote me a prescription and there and then.

My GP now prescribes it, but only once the gynaecologist write to him to confirm I need it. 😵‍💫

It’s not different. That is systemic HRT. It is unlikely to help vaginal atrophy symptoms alone.

I said I was worried about v atrophy as my clitoris has virtually disappeared and this is what I came away with.

BellsAllTheTime · 06/06/2026 22:57

I had this problem start recently (after a few years of low libido).

My menopause specialist suggested Intrarosa - which is DHEA (precursor to estrogej and testosterone) pessary you insert vaginally. It's not absorbed systemically, delivers hormones locally, so I don't think you need to be on systemic HRT with optimised estrogen levels to use it (although I am on systemic HRT already).

It's not available on the NHS in my area, but it is in some areas.

I can't comment on effectiveness as I've not used it yet. She thought it would help significantly with the specific loss of orgasm.

I don’t think he’s given you enough. Local oestrogen cream needs adding to your regimen.
