Sequential HRT & skipped a period but had PMS

Hello 👋

I have been on sequential HRT for 6 months. Initially Evorel 50 patches + 14 days utrogestan. Then increased patches to 75 about 3 months ago.

Completely eliminated my night sweats and insomnia, migraines really improved. Slight blip at 3-months in, then I went up to 75mg patch and all was well again.

Prior to HRT I was having regular but very short (21 - 24 day) cycles, very light. After starting HRT they got a bit heavier, but came on exactly the same day they would have if I hadn't started the utrogestan. It started when I hadn't finished the utrogestan (because I was having a short cycle), I just carried on taking the utrogestan and stopped after 14 days.

The last 2 months have been slightly longer cycles, back to 28 days. Then this month, no period at all, not even the hint of any bleeding. But at the time I was due to have a period I got all the PMS stuff (cramps, moods, sore boobs, waking up at night and hot flushes came back, migraine every day for a week), but no actual period. Not even a very light one.

I started the utrogestan 2 days earlier the last couple of months because my specialist suggested I use more days of utrogestan to try and make the periods lighter, but I have short cycles so it probably was before the mid-point of my cycle I started the utrogestan.

Could it be that I blocked ovulation by starting it earlier ? My own cycles do seem to override the utrogestan, so perhaps I've just moved to a step later in the process ?

Ive gone back to hot flushes, migraines and waking in the night. It's very frustrating and I'm very confused!
