Post menopause bleeding - large blood clots

Aged 54 on continuous HRT since last November. Prescribed 75 Estraderm patches, 100 Utrogestan though this has recently been increased to 200 and 10 Vagirux pessary. Started having persistent bleeding about a month ago. Consulted with GP hence increased Utrogestan and put on the 2 week referral pathway. Went for a transvaginal ultrasound which showed endometrial thickness of 4.5 mm and a small fibroid. Went for a gynae consultation at hospital (no exam just a chat and options) and they want to go a hysteroscopy and biopsy later this month. I have opted for a GA. Very happy with how quickly the referral pathway has gone and lovely staff at hospital.

However, I am bleeding continuously and quite heavily at times and also sometimes passing some large blood clots/tissue. Bit worried about that. Has anyone else had this and could you let me know if this is to be expected with increased utrogestan?
