Has HRT stopped working?

I've been on Estrogel and Uterogestan/Gepretix for about 4.5 years (I'm 51) and overall it has been keeping symptoms at bay, or at the very least manageable. The main symptoms I had pre-HRT were headaches, fatigue, flushes and general meh.

I'm still having periods, so I am taking 200mg prog vaginally for 12 days every month. I don't like it very much but it's a price worth paying to get rid of symptoms.

Weekend before last, while having period, the headaches came back. I know when they are hormonal as they feel like pressure rather than pain, I get nausea and need to lie down. Passed after a couple of days, then the most awful anxiety rollercoaster began. Now headaches are back too.

It does all feel hormonal, and I wondered if it is my hormones having a last freak out before giving up! I upped gel to 4 pumps a day this week to see if it helps but no change yet. I don't think my GP will be clued up enough to have any answers but someone here might have had the same happen at this stage of peri?

And to add to list of woes, I feel absolutely exhausted today with brain fog, but the anxiety has passed.
