Where would I find the revised recommendations that progesterone should be taken in proportion to amount of oestrogen?

I am on 4 pumps of oestrogel and 200mg of utrogestan on days 15-26 of cycle. I have read that recommendations have changed and that it's now recommended for women on higher doses of oestrogen to take more progesterone. Some women have said that they've been contacted by their GPs following this change to up their dose.

I have been having some breakthrough bleeding and am concerned this could be due to not having enough progesterone. I have an appointment with my GP Friday and would like to discuss the revised recommendations with him, but I can't however find them anywhere. Does anyone have a link to these revised recommendations from a trustworthy site (eg BMS or NICE).

If you search Mumsnet using the search facility you should find it in this part - Menopause. I've linked to it many times including a week ago on someone's thread, and am pretty sure that I started a new thread on it some time ago if you scroll back through the posts.
